Email to Homebound Services
This service provides needed information to those customers of the Library who, by medical necessity, are restricted to their homes on a long-term basis. A letter from the personal physician on letterhead is required to be sent to the attention of: LVCCLD/Homebound Services, Outreach Specialist, Sylvia Riesselmann (email@example.com) fax #: 702.507.6333.
The letter must state that, in the opinion of the physician, the customer is indeed homebound. The letter should also state the length of time the customer is expected to be homebound.
This service is not intended to serve those with minor injuries which are temporary in nature.
Once a person is identified as no longer being homebound the Department is to be notified and the patron will be removed from homebound status.
